ST. LOUIS, MO — SSM Health Care – St. Louis is pleased to announce the appointment of Naveed Razzaque, M.D., as medical director for SSM Senior Services Continuum of Care. Razzaque will serve as a liaison with physicians who provide care across the senior services continuum, both for SSM Health Care and in the community.
Razzaque will help define the service standards for providing exceptional health care for seniors while helping identify and develop a panel of specialist physicians and providers. He will also provide leadership on clinical projects to continue improving SSM Health Care’s medical treatment for older adults.
Razzaque’s experience provides unique insight into physician roles across skilled nursing facilities, clinics and inpatient settings helping him understand the complexity of coordinating patient care across these settings. He has been with SSM Health Care – St. Louis for 24 years, is board-certified in internal medicine and currently serves patients at SSM DePaul Health Center. He earned his medical degree from King Edward Medical College in Lahore, Pakistan and completed his residency in internal medicine at the University of Missouri – Kansas City. He is also a Fellow of the American College of Physicians.
Razzaque’s many credentials include serving as the secretary for the Missouri Board of Nursing Home Administrators with the Department of Health and Senior Services, a member of the American Medical Directors Association and a member of the Best Practices Consortium through the sate of Missouri. In addition to work for SSM Health Care – St. Louis, he serves as a medical director for three nursing homes in North St. Louis County.
