The people of Senior Care Services at St. Clare thoughtfully coordinate the details of your senior loved one’s health visit. Our helpful, dedicated staff works with retirement, assisted-living and long-term care communities to streamline outpatient diagnostic scheduling for seniors. The staff compassionately escorts patients to the appropriate appointment location, when needed.

ANTICOAGULATION SERVICE
Anticoagulants are medications used to slow blood clotting in some patients with heart conditions. The Anticoagulation/Coumadin® Service at St. Clare has specially trained pharmacists who assist patients in the management of anticoagulation therapy, including warfarin (Coumadin®) therapy. Patients receive:
• Easy access and a simplified registration process
• Testing by finger stick
• Immediate test results/dosage adjustment
• Ongoing patient education
• Clinician consultation
• Monitored anticoagulation management
• Communication with the patient’s physician as needed.
A physician referral is required for participation in this service. Upon receiving
a referral authorization, patients will be scheduled for an initial visit.

TRANSPORTATION
St. Clare Health Center offers convenient transportation for patients residing in the hospital's service area. Courteous, well-trained drivers will pick up patients and transport them to St. Clare -- or to the St. Francis or St. Joseph medical office buildings on campus. Upon arrival, St. Clare staff escorts patients to the appropriate campus location. A wheelchair-accessible van is available for those with special needs. (There is a $5 round-trip charge to the hospital and a $10 round-trip charge to the medical office buildings.)
