Download WLI referral form. Once the form is complete then you can:
- Call our dedicated physician referral line at 314-344-7199 and provide information or,
- Send the referral form information via email to Patsy_Beeson@ssmhc.com or,
- Fax completed referral form to 314-344-6801
After the patient referral information is received by the Weight-Loss Institute, then your patient will be contacted within 24 hours.
If you are a primary care physician and you know your patient is seeking a surgical weight-loss option, please download and complete “Primary Care Physician Request Form” along with the WLI referral form. This information is required for pre-authorization of insurance.
