Founded by Mother Agnes Hazotte and the Congregation of Sisters of St. Agnes, SSM Health St. Agnes Hospital - Fond du Lac treated its first patient in 1896. The 146-bed hospital has grown into both an inpatient and outpatient facility providing innovative community and regional health care services to the greater Fond du Lac area.

Visitor Guidelines
- SSM Health has discontinued its universal masking policy.
- Masks are no longer required, with the exception of those who have known or suspected COVID-19 or other upper respiratory symptoms.
- Patients will continue to be screened for upper respiratory symptoms, and masks will be provided for those who need one.
- We support patients and visitors who choose to wear a mask.
Additional hospital visitor guidelines:
- Patients who are COVID-positive, presumptive positive, or under investigation for a COVID infection may have visitors. Please check in with nursing staff before entering the room.
