at SSM Health St. Clare Hospital – Baraboo
GoldenCare is a free program, available to Wisconsin residents, 60 years of age and older. You do not have to be a patient at SSM Health St. Clare Hospital – Baraboo or SSM Health Dean Medical Group to take advantage of the many benefits and discounts of GoldenCare senior membership.
Apply online or stop in the GoldenCare office at SSM Health St. Clare to sign up. You’ll receive your GoldenCare membership card in the mail. You can start taking advantage of your GoldenCare senior program benefits right away.

- 10% discount on meals at SSM Health St. Clare Hospital - Baraboo cafeteria
- 10% discount at SSM Health St. Clare Hospital – Baraboo gift shop (exclusions apply)
- 15% discount on any complete pair of eyewear (lenses and frames) from SSM Health Davis Duehr Dean Eye Care
- No other discounts can be applied.
- 10% discount on over-the-counter cash purchases from Home-Town Pharmacy (clinic location only) with a maximum of $3 per item
- This discount does not cover Nicorette Gum.
- Discount does not apply if prescriptions are covered by insurance.
- 20% discount on cash purchase of medical equipment and supplies at SSM Health at Home
- Discounts only apply if there is no insurance coverage.

When taking advantage of the discounts offered, please limit yourself to a maximum of $50 annually as required by OIG regulations.
