Hyperlexia Evaluation
Phone Consultation
Consultations are geared toward general recommendations and solutions based on hypotheticals. A true diagnosis isn't valid remotely and will not be billed as such. A phone consultation is not the establishment of a patient/provider relationship.
- A phone consultation fee is $300 per hour.

- Read and sign our consultation contract
- Email the completed contract to paige.pergande@ssmhealth.com at SSM Health Behavioral Health.

In-Person Evaluation
If you would like to schedule a multidisciplinary hyperlexia evaluation for your child in Fond du Lac, WI for treatment recommendations:
- Multidisciplinary evaluations are offered three times per year.
- The fee is $4,500.
- Travel and transportation is not included.
- Evaluations are arranged through SSM Health Behavioral Health.
