  • Are you under the age of 35?
  • Are you trying to schedule a six-month follow-up exam?
  • Are you experiencing breast problems, such as a lump, nipple discharge or other known changes?
  • Have you been diagnosed with breast cancer in the last five years?
  • Do you have breast implants?
  • Have you had breast surgery within the last year?
  • Are you trying to schedule a six-month follow-up exam?
