Sun Prairie Area School District & City Wellness Clinic

The Sun Prairie Area School District and the City of Sun Prairie are pleased to offer an employee wellness clinic to all regular part-time/full-time employees, their spouses, and their dependent children (ages 2+). Employees do not have to be a part of the District’s/City’s health insurance plan in order to receive services at the clinic. Seasonal, temporary, and substitute employees; and elected officials are not considered regular part-time/full-time employees.

Our team works with your primary care provider to offer personalized care. During your visit, you will receive personalized time with a dedicated SSM Health professional with access to your existing medical records. We provide assistance in scheduling appointments for other areas of care and share important information with your primary care provider.
