EpicCare® Link™

Securely connecting you to your patient's health data.

SSM Health's EpicCare® Link™ is a secure, web-based portal. It connects community providers* with information stored in SSM Health's electronic medical records.

EpicCare® Link™ provides continuity of care for the patients we serve. As an EpicCare® Link™ user, you can:

  • View a patient’s clinical history, test results, clinical reports, referrals, and images.
  • Receive real-time notifications when a patient is admitted to a SSM Health hospital.
  • Securely message SSM Health providers about your patient's care.

EpicCare® Link™ is for providers outside of SSM Health, including:

Requesting access to SSM Health's EpicCare® Link™:

  • If your facility does not have access to EpicCare® Link™:
    • A physician, practice manager, director or legal signing authority will need to complete the new site request access form.
    • As part of the new site request, you must request at least one user account to be the site administrator.
      • Site administrators request and manage new user accounts, passwords, as well as deactivate users.
  • If your facility already has acccess to EpicCare® Link™:
    • You will need to contact your site administrator to request a new user account.
