Residency Requirements & Application Procedure

Requirements for Acceptance

Applicants must be graduates of an ACPE-accredited School of Pharmacy (PharmD preferred) and be eligible for licensure in the state of Oklahoma. A complete application includes:

• Curriculum vitae
• Three letters of recommendation
• Official pharmacy school transcript
• Letter of intent 
  • The letter of intent should highlight the applicant’s career goals, motivation for pursuing a PGY1 Pharmacy Practice Residency, and specific interest in completing the residency at SSM Health St. Anthony’s Hospital. Additionally, please select one of the following SSM core values and speak to how you exhibit that value through your daily life: compassion, respect, excellence, stewardship, or community.
