Requirements for Acceptance

• ASHP Match Code: 316513

Candidates must graduate from an ACPE-accredited School of Pharmacy and be eligible for a license in the state of Illinois. Applications are eligible to apply in PhORCAS through January 7th, 2026. A complete application includes:

• Curriculum vitae (CV)
• Three letters of recommendation
• Official pharmacy school transcript
• Letter of intent

The letter of intent should include future career goals, motivation for pursuing a PGY-1 pharmacy Residency, and specific interests at SSM Health Good Samaritan Hospital
