• Candidates must graduate from an ACPE-accredited School of Pharmacy and be eligible for a license in the state of Wisconsin.
• Applicants must meet criteria defined by the ASHP residency accreditation standards to be considered for a position.
• Complete an application and at minimum submit the following information to PhORCAS:
  • Letter of intent
  • Curriculum vitae (CV)
  • Pharmacy School Transcripts
  • Three letters of recommendation
