How to make a maternal fetal referral:

To make a referral: print and fill out the maternal fetal medicine referral form and fax it, along with supporting documents, to 314-622-6472:

• Maternal fetal medicine referral form
• Patient medical records
• Patients insurance card, front and back

What is the TeleTouch pregnancy connect referral process?

1. Fax the following information to 314-951-7216:
   • MFM Telemedicine services order
   • Prenatal records including ultrasound reports, consent form, and labs
   • Insurance card
2. Call 800-237-6753 to schedule appointment
3. Referring site will provide patient with appointment date, time, and location

Upon completion of the patient encounter, the consultation and ultrasound report will be faxed to the referring provider office.

Please note, if any unanticipated or urgent concerns are identified the Maternal and Fetal Medicine physician will request to speak with the referring physician during the patient encounter.

If you are a primary care physician and you know your patient is seeking a surgical weight-loss option, please download and complete the Primary Care Physician Request Form as well as filling out the the referral form. This information is required for pre-authorization of your patient’s insurance.
