Healthcare Power of Attorney Sessions

SSM Health is hosting free health care power of attorney sessions. These one-on-one discussions are led by experienced social workers who will help you understand your options and complete your documents.

In Madison

Sessions are held in the Resource Room, just across from the pharmacy at SSM Health Dean Medical Group, located at 1821 S. Stoughton Road.

In Fond du Lac

Sessions are held at SSM Health St. Agnes Hospital, located at 430 E. Division Street in Fond du Lac.

Registration is not required.
