Diabetes Self-Management Education
If you have been diagnosed with diabetes, SSM Health has many educational programs to help you learn how to better manage your disease and achieve a healthy lifestyle. The program provides education according to the American Diabetes Association National Standards for Diabetes Self-Management Education.
Without proper treatment and management, diabetes can lead to serious nerve, heart, eye or kidney problems, but this does not have to happen to you! A little knowledge about diabetes can go a long way in keeping you healthy while living with this serious disease. Diabetes Self-Management Services
Our diabetes self-management education programs are appropriate for anyone diagnosed with type 1 diabetes, type 2 diabetes, gestational diabetes or pre-diabetes. Diabetes Support Group
Many of our locations also offer a free, monthly diabetes support groups. Topics vary each month, but these groups are a great way to share your questions and experience with others living with diabetes. How to Participate
To participate, contact your physician and request an order for both diabetes education and nutrition education. Our services are provided by a highly trained staff of professionals and is under the guidance of an advisory board. We will work with you and your doctor to provide the care, support and training you need.
Our diabetes education services are covered by Medicare and most private payers. Medicaid does not cover any education. However, Medicaid and self-pay patients may be eligible for financial assistance.
