The SSM Health Hospice & Home Health Foundation is a non-profit 501 (c)(3) organization established to support the services of SSM Health at Home.

While ours is a vast organization serving communities in the greater St. Louis area, Southern Illinois and Oklahoma, all contributions remain local and are invested back into the community in which it is received.

Our Mission: “Through our exceptional health care services, we reveal the healing presence of God.”

We exist to help fulfill our Mission of providing exceptional health care and revealing God’s healing presence to the people we serve. We do this not only by providing a financial safety net for our most vulnerable patients and families, but by providing an emotional safety net as well.
