Fall Prevention Program

SSM Health at Home provides comprehensive care for patients who are high risk for falling. Per the CDC (Centers for Disease Control and Prevention), one out of three older people fall each year, but less than half tell their doctor. Chances of falling doubles once you have already taken a fall.

Fall Risk Factors

Preventing falls is much more effective than treating them. While falls can happen anywhere they most often occur at home. Risk factors for falling include:

  • Muscle weakness
  • Poor balance
  • Drop in blood pressure
  • Foot problems / improper footwear
  • Vision issues
  • High number of medications
  • High risk medications
  • Sensory or sensation issues
  • Memory or cognitive issues
  • Recent illness or surgery
  • Previous fall
  • Environment - obstacles, lack of safety equipment, poor lighting
