Located inside the SSM Health Outpatient Center at 2850 O’Keeffe Avenue in Sun Prairie, Wisconsin, SSM Health Davis Duehr Dean Eye Care provides care for your family’s vision near the corner of Reiner Road and U.S. Highway 151. Eye care services are on the second floor, and Optical Shop is on the first floor.

Check in online with MyChart

Save time before your visit by completing your pre-visit tasks through MyChart eCheck-in up to seven days before your appointment.

When you arrive, you can skip the registration kiosk and go directly to the Room Assignment Station in the lobby. A team member will guide you to your exam room.

Check in when you arrive

If you prefer to register in person, use one of the registration kiosks located in the lobby when you arrive for your appointment. Registration kiosks allow check-in beginning 15 minutes before your scheduled arrival time. You can also view check-in instructions or review our frequently asked questions for guidance.

Get your room assignment

After completing check-in, visit the Room Assignment Station located next to the registration kiosks.
