The BRIDGE Clinic at SSM Health Saint Louis University Hospital

The BRIDGE (Bridging Recovery, Informing Decisions & Guiding Empowerment) Clinic provides a one-time appointment with a physician or advanced practitioner if you are unable to see your primary care doctor within 14 days after leaving the hospital.

The BRIDGE Clinic works to support patient care transitions, recovery, and reduced risk of hospital readmission and additional emergency department visits. Additionally, there is a pharmacist on staff who works with members of your healthcare team to ensure the medicine you are prescribed is effective, safe, and affordable. All patients receive the best possible care for their health needs and are supported in their transition from hospital discharge to on-going primary and specialty care. 

The clinic is located on the second floor of Bordley Tower at SSM Health Saint Louis University Hospital. The entrance can be found off Vista Avenue and parking is available across the street in the parking garage.
