SSM Health Day Institute in O'Fallon, IL is located off of Highway 50 near Green Mount Road. SSM Health Day Institute offers comprehensive outpatient rehabilitation specifically designed to maximize physical and functional abilities and independence. Programs focus on a wide range of physical, cognitive, emotional, behavioral, social and psychological challenges, as well as, self-care issues that an injury or illness may have caused.

Our team of rehabilitation experts works closely with you and your family to develop a treatment plan that addresses your individual needs and goals. Together, we seek to build on existing strengths and skills and use environmental modifications or behavioral strategies to overcome any deficits. As part of your program, we also practice community reintegration skills, assess pre-vocational and /or school abilities and address any return to driving abilities.
