SSM Health Homeward Bound

SSM Health Homeward Bound partners with St. John’s Place in Overland, Missouri, a quarter-mile north from Brown Road, to provide rehabilitation services seven days a week to individuals who either do not need or qualify for inpatient rehabilitation, but are not quite ready to return home after a serious injury or illness.
 
SSM Health Homeward Bound is part of an integrated network of rehabilitation services offered by SSM health Rehabilitation Hospital. Patients have convenient access to coordinated and compassionate care in a dedicated rehab unit for every rehabilitation need, as well as access to the services of a skilled nursing facility.
 
SSM Health Homeward Bound offers the transitional care to get you back home after your hospital stay. Our goal-oriented rehab program is designed for patients who are recovering from a serious illness or injury such as joint replacement, stroke, neurological conditions or spinal cord injury, and require 24-hour nursing care.

Therapists who specialize in lymphedema are on staff. Pain management, wound care and memory care specialists, as well as a full-time dietician are also on staff at St. John’s Place.
