SSM Health Prescription Center, located in Fort Atkinson, Wisconsin, accepts prescription refill requests 24 hours a day by phone and via MyChart. Refills may be picked up at any SSM Health Pharmacy or Prescription Center location or mailed to your home. A member of our staff will call you when your order is ready if you request. Our pharmacists are always ready to answer questions, provide assistance and make sure your medications are exactly right.

SSM Health Prescription Center accepts most insurance plans, including Dean Health Plan, Navitus (State of WI Employees - ETF), Medicaid, Medicare Part D Plans, Humana, Express Scripts and CVS/Caremark.

If you need to transfer your prescriptions to SSM Health, simply call the pharmacy or prescription location and we'll take care of the details.

If you need a copy of your SSM Health Pharmacy records, submit a completed Authorization to Release Pharmacy Records form.

Please note: This Prescription Center is closed on the following holidays: Labor Day, Thanksgiving Day, Christmas Day and New Year's Day.
