SSM Health hospitals in Wisconsin earn high scores from CMS

Two SSM Health hospitals in Wisconsin have earned a Centers for Medicare and Medicaid (CMS) 5-star Overall Hospital Quality Star Rating for 2026, while four others earned a CMS 4-star rating this year. All of SSM Health’s CMS rated hospitals in Wisconsin increased or maintained their rating from 2025. This is a testament to the health system’s commitment to providing safe, exceptional, and patient-centered care.

SSM Health Wisconsin locations that earned CMS 5-Star ratings were:

  • SSM Health Monroe Hospital
  • SSM Health Waupun Memorial Hospital

SSM Health Wisconsin locations that earned CMS 4-Star ratings were:

  • SSM Health St. Agnes Hospital – Fond du Lac
  • SSM Health St. Clare Hospital – Baraboo
  • SSM Health St. Mary’s Hospital – Janesville
  • SSM Health St. Mary’s Hospital - Madison

To determine a star rating, CMS measures hospitals based on sets of quality measures, including mortality, safety of care, readmissions, patient experience, and the timeliness and effectiveness of care.

The CMS 5-Star Quality Rating System serves as a valuable tool to help patients and families compare hospitals and make informed decisions about their health care. Only 384 US Hospitals achieved this status in 2026.
