PGY2 Ambulatory Care Pharmacy Residency

SSM Health Dean Medical Group operates over twenty primary care clinics throughout the greater Madison area. Our ambulatory care pharmacy team spans multiple specialties, with pharmacists embedded in interdisciplinary medical teams or working remotely to ensure consistent, high-quality care across the region.
Pharmacy teams function under collaborative practice agreements, granting them autonomy to make clinical decisions within their scope of expertise and positioning them as integral members of the healthcare team. Our ministry is deeply committed to value-based care, with a strong emphasis on improving population health—particularly within underserved communities.
Residency program overview
The PGY-2 resident will rotate through our ambulatory pharmacy teams gaining valuable insight into management of a variety of chronic conditions via extensive collaborative practice agreements. The resident will have nearly a year-long longitudinal primary care learning experience where they will have an opportunity to build their own practice and establish meaningful relationships with the patients they care for. This residency program will provide an opportunity to collaborate with various interdisciplinary teams and serve a diverse patient population. The primary goal of this residency program is to prepare the resident for advanced independent practice as an ambulatory care pharmacist amongst a variety of disease states.
The resident will gain exposure to a wide range of ambulatory pharmacy services, including:
- Longitudinal Primary Care Clinic
- Cardiology Clinic
- Oncology and Specialty Infusion Clinic
- Infectious Disease Outpatient Clinic
- Population Health
- Anticoagulation Clinic
- Administration: Leadership and Practice Management
Certification and teaching opportunities:
- Teaching Certificate Program through University of Wisconsin School of Pharmacy (optional, but encouraged)
- Physical Assessment for Pharmacists Certificate through Medical College of Wisconsin
- Precepting of APPE/IPPE students
