Cystic Fibrosis Transition Program

The cystic fibrosis transition program is designed to help patients overcome the hurdles of childhood and adolescence with a chronic disease. It also helps patients develop into independent adults who are informed about CF and are able to care for themselves and manage all aspects of their health care.

Pulmonologists from the fully accredited SLUCare Adult Cystic Fibrosis Center at SSM Health Saint Louis University Hospital attend pediatric CF visits at SSM Health Cardinal Glennon. This gives young patients an opportunity to meet new doctors in a setting that’s comfortable and familiar to them. It also gives our pulmonologists the opportunity to meet patients, learn their history, and develop a plan for continuing their care into adulthood.
