About Our Hospital
Founded by Mother Agnes Hazotte and the Congregation of Sisters of St. Agnes, our 146-bed hospital has grown into a state-of-the-art facility providing health care services to the greater Fond du Lac area, including:
- Comprehensive heart and vascular care at SSM Health Dale Michels Heart & Vascular Care.
- Immediate care for stroke patients at our Joint Commission certified Primary Stroke Center.
- Inpatient physical rehabilitation for patients with serious conditions.
- Private, spa-like birthing suites to welcome your baby into the world.
- 24/7 emergency care at our designated Level III Trauma Center.
Helpful Hospital Resources for Your Visit
The chapel at SSM Health St. Agnes Hospital - Fond du Lac is open 24 hours a day. People of all faiths are welcome to visit for meditation, prayer, and scheduled religious and spiritual health-related programs.
Communion
Communion is distributed to Catholic patients who wish to partake. Patients may also request communion from their personal clergy.
We have chaplains available to offer support and comfort at times of illness, injury and crisis. In keeping with our mission, chaplains honor the sacredness and dignity of all persons at every stage of life, and offer prayer and a listening presence to people of all faith traditions. When requested to do so, spiritual care staff will contact the patient's own clergy.

Visitor Guidelines & Helpful Phone Numbers
Since visitors can often be the best medicine for patients, we have open visiting hours (some specialized units of care may have more specific hours for visitors, please check at the nurses station).

Patients who are COVID-19 positive, presumptive positive, or under investigation for a COVID-19 infection may have visitors. Please check in with nursing staff before entering the room.
