How does remote patient monitoring work?

It starts with a treatment plan created by a patient’s physician and SSM Health care team. Patients then track predetermined vital health stats at home. The information is monitored and if there are any concerns, SSM Health staff will contact the patient to discuss next steps.

Your care team will send monitoring devices, such as a scale or a blood pressure cuff, directly to your home. Your care team will teach you how to use these devices so you can easily take your vitals and respond to symptom surveys on a daily basis. Then, your care team will receive this information automatically, reviewing and following up on it as needed.

Through this program, you will also have the opportunity to connect with your nurse through video visits where you will learn more about how to manage your health conditions at home.

You will also learn more about managing your health conditions so you can feel confident continuing your health care journey at home even after you complete the program.

Talk to your health care provider about SSM Health’s remote patient monitoring program. Your health care provider can help determine if you are eligible and send a referral to our program if you meet program criteria.

How much does it cost?

Remote patient monitoring services are included in many insurance plan coverages. Expected coverage will be reviewed at the time of the referral. Patients are encouraged to reach out to their insurance providers for more information.
