SSM Health operates three hospice residential facilities in Wisconsin. The SSM Health Hospice Home of Hope (Fond du Lac), SSM Health Hospice House (Baraboo) and SSM Health Monroe Hospice Home are available to hospice-eligible patients whose symptoms can be managed in a residential setting but are unable to have those needs met at home.

SSM Health’s hospice residential facilities feature a team of caring individuals trained to meet the physical, spiritual, social and emotional needs of patients who have a limited life expectancy. Through skillful pain and symptom management and the support of a committed team of professionals and volunteers, every effort is made to provide comfort and enhance the quality of life for patients and their families.
While staying at one of our hospice residential facilities, residents receive all the in-home hospice services plus meals, medication administration, laundry and housekeeping. Our facilities are staffed 24-hours a day.
