Preparing For Your Appointment
Drivers in need of a DOT-certified test or physical should bring the following items to ensure successful completion of the exam:
- Drivers license
- List of medications
- Bi-PAP/CPAP report, if applicable
- Physician’s written release to drive a commercial vehicle if you have had a surgery, major illness or injury within the last five years
