We are a Medicare and Medicaid-certified provider of skilled nursing, occupational/physical/speech therapies, medical social services, and home health aides.

SSM Health at Home conducts several background checks and verifies licenses and certifications to ensure our patients feel comfortable and safe with our in-home care team.

Our intake representatives are available to help determine if the patient meets Medicare/Medicaid or private insurance criteria. Please contact us with any questions you have about eligibility.

SSM Health at Home assigns a nurse care manager to each patient who coordinates care according to a written plan of care with all other disciplines, including the patient, the patient’s physician, and the caregiver(s). Our staff will educate caregiver(s) on disease processes and care for each patient. They also teach the patient self-care techniques and lifestyle changes to help them remain as independent as possible after they are discharged from home health services.
