How does remote patient monitoring work?

t starts with a treatment plan created by your physician and SSM Health at Home care team. Patients then track predetermined vital health stats at home. The information is monitored and if there are any concerns, SSM Health at Home staff will contact the patient to discuss next steps.

Who can remote patient monitoring help?

Patients with a primary diagnosis of congestive heart failure are currently eligible for remote patient monitoring. Please contact your health care provider or the SSM Health at Home team for more information.
