Important Update: Effective August 1, 2025, SSM Health will no longer participate in the Multiplan (Claritev) /PHCS network. Unless it is a medical emergency, services at SSM Health facilities will not be considered in-network, and your benefits and coverage will be affected on or after August 1, 2025. If you have questions, please contact MultiPlan (Claritev)/PHCS at 1-800-950-7040 for assistance. This is the number located on the back of your insurance card.
