SSM Health has been working to meet the needs of Oklahoma residents since our founding sisters opened the first hospital in the Oklahoma Territory 125 years ago. In 2023, SSM Health’s ministry in Oklahoma served the surrounding communities through a tertiary hospital in downtown Oklahoma City, a specialty orthopedic hospital, acute care facilities in Shawnee and Midwest City, a hospital campus in Seminole, five healthplexes, five urgent care centers, six managed hospitals and more than 350 employed medical group providers located in 81 clinic locations. Together, we are committed to improving the lives and health of our patients and communities, with special focus on the region’s most pressing health needs, including behavioral health, food insecurity, and access to primary care services.

Health Coaches for Hypertension Control

Empowering people to positively impact their health

Health Coaches for Hypertension Control, a program developed at Clemson University and facilitated by the community health team, empowers members of the Oklahoma City community to actively manage their hypertension and prevent related issues such as heart attack and stroke.

The program provides comprehensive support that includes personalized action plans, group sessions, phone counseling, and access to broader community resources to foster sustainable lifestyle changes.

Participants receive a blood pressure monitor, cookbooks, and a community resources directory provided by SSM Health St. Anthony Hospital - Oklahoma City and SSM Health St. Anthony Hospital - Midwest. Since its inception, 82% of participants completed the eight-week program.

In addition, 89% of the participants reported improved knowledge regarding nutrition, physical activity, stress management, and blood pressure disease, and 84% reported enhanced skills in setting goals, creating action plans, and effectively communicating with their health care providers.
