The SSM Health Maternal Access Center provides a direct path to expert maternity care for Missouri and Illinois OB/GYNs.

Maternal Fetal Referral Information
To make a referral: print and fill out the maternal fetal medicine referral form and fax it, along with supporting documents, to 314-622-6472:
- Maternal fetal medicine referral form
- Patient medical records
- Patients insurance card, front and back
TeleTouch Pregnancy Connect Referral Process
- Fax the following information to 314-951-7216:
- MFM Telemedicine services order
- Prenatal records including ultrasound reports, consent form, and labs
- Insurance card
- Call 800-237-6753 to schedule appointment
- Referring site will provide patient with appointment date, time, and location
Upon completion of the patient encounter, the consultation and ultrasound report will be faxed to the referring provider office.
Please note, if any unanticipated or urgent concerns are identified the Maternal and Fetal Medicine physician will request to speak with the referring physician during the patient encounter.
